Trademark Checklist
The following information is necessary for Patents+TMS to prepare and file a trademark application on your behalf:
- The mark to be registered (word, phrase, logo, design, or combination, etc.)
- Description of goods or services on which mark is used or being used
- Name of Applicant
- Applicant's Address
- Applicant's entity (individual, corporation, partnership, etc.)
- State of Incorporation (if applicable)
- Is the mark currently in use on or in association with the goods and/or services identified above? YES or NO
- If YES, date(s) of first use (anywhere/in commerce) of mark for each class of goods and each class of services
- If NO, trademark application may be filed based on an INTENT TO USE the mark. A subsequent Statement of Use and appropriate fees are required following allowance by the U.S. Trademark Office prior to registration of the mark. A trademark may NOT BE REGISTERED unless USE of the mark is shown on or with the goods and/or services for which registration is sought.
